Enrollment Services

Our knowledgeable and experienced Enrollment Services Department staff will help you understand and navigate the enrollment process for health care programs such as Medi-Cal, Covered California, CalFresh, Medicare, and HealthPAC.

Our medical services are also available on a sliding fee scale based on income and family size. Regardless of your income, insurance status, or ability to pay, we will work with you to help you get the care you need. No one will be denied access to services due to inability to pay.

Health Insurance options are changing fast. New health insurance laws will affect different people in different ways. Axis Community Health's Certified Enrollment Counselors can answer your questions about health insurance and help get you covered.

Medi-Cal enrollment is open all year. Medi-Cal is California's Medicare program. Our Enrollment Counselors can help new patients apply for Medi-Cal, and help current Medi-Cal patients renew their coverage.

Covered California may offer low-cost options. If you are not eligible for Medi-Cal, you may be able to get coverage through Covered California. We can help you fill out your online application and help determine if you are eligible for financial assistance to cover the cost of a health plan.  

Covered California Open Enrollment runs from October 15, 2018 to January 15, 2019. Now is the perfect time to explore your options with Covered CA, and open an application. Visit CoveredCA.com or contact Axis at 925-462-1755 for more information.

Individuals who experience a major life event may qualify for "Special Enrollment," and enroll at any time of the year. Common examples of qualifying events include marriage or entry into domestic partnership, birth of a child or adoption, job loss, change in immigration status, relocation to or within California, and many other situations. CalFresh is a nutrition program that helps families get healthy food. When a family or individual signs up, they receive a standard debit card that they can use to buy food at grocery stores and markets. Every month, the card is reloaded with a specific amount based on the family's size and income. It's easy to enroll - just call 925-462-1755 to make an appointment with one of our enrollment staff.

We offer enrollment appointments at each of our three medical clinics listed above. While we do accept "walk-ins," we encourage applicants to call ahead and check for availability.
