CHIEF MEDICAL OFFICER

Dr. Dawnell Moody, DO

As Chief Medical Officer, Dr. Moody makes sure our patients receive quality care – including her own. She knew she wanted to work with children during her first visit to a pediatric clinic in medical school – “it just clicked,” she says. Now, working with children is the highlight of her day; she loves to see her patients grow and thrive. She also knows how much parents care about their children, and makes sure to take time to address their concerns and connect them with other helpful community resources.

Dr. Upama Barua, MD

As a child helping her grandfather at a local free clinic, Dr. Barua got an early start on her career in medicine. “I want to understand my patients’ lives,” she explains, “so that together we can find solutions that work outside the exam room.” Dr. Barua knows the importance of research and evidence-based care, and stays up-to-date with new developments. She completed a year-long geriatric fellowship before joining Axis, and is excited to be living and working in Pleasanton.

Dr. Anuja Garg, MD

Dr. Garg believes that patient health education is just as important as treatment, and enjoys working at a clinic that values both. She makes a point of identifying each patient’s largest health concern, and to spends time explaining how and why symptoms occur. She has a special interest in endocrinology, including thyroid disease and diabetes, and enjoys helping her patients successfully manage these chronic diseases.

Dr. Gunjan Munjal, MD

With two doctors for parents, Dr. Munjal became excited about medicine at a very early age. She aims to help her patients to feel at ease, and strives to make the exam room a place where they can feel safe to express their concerns. Dr. Munjal is enthusiastic about community health, and worked at a community hospital for over three years before joining Axis. It was there that she developed a special interest in caring for the elderly.

Dr. Kusuma A. Hampapur, MD

Dr. Hampapur grew up in a culture of service and has always wanted to bring healing and help to others.  She is eager to support her patients in learning to care for themselves physically and emotionally. To accomplish this, she follows up with patients to make sure they are getting better, staying healthy, and are happy.

Dr. Ronald Yang, MD

Growing up, Dr. Ronald Yang had such positive experiences with physicians, it led him into a medical career that has spanned over 30 years. “Being healthy is a happy and joyous thing”, he says, “but not everyone can get the care they need.” Dr. Yang has practiced medicine in Asia and America and has seen the effects of healthcare inequality. “Community health is so important because it gives those who need healthcare a chance to thrive and lead healthy lives,” says Dr. Yang. He is currently Axis’s urgent care provider and is not yet accepting patients.

Dr. Sudha Chadalawada, MD

Dr. Chadalawada finds each day at Axis unique. She enjoys working with patients of all ages and different medical needs. For her, the variety is energizing. She strives to see her patients’ health improve over time. As a bilingual Telugu speaker, she is eager to use her language skills whenever possible to provide care their home language.

Dr. Debra Cheng, DO

As a child of immigrants, Dr. Cheng became aware of the health needs of non-English speaking community members while growing up. It made her realize she wanted to go into medicine, particularly family medicine where she enjoys the challenge of needing to know a lot about many things. She believes it is important to know what is going on with her patients that affects their lives and health, which requires being open and listening. She is passionate about the Axis mission of serving the community and excited to be part of the team.

Dr. Michelle Cortes-Surdilla

“I grew up in Livermore,” explains Dr. Cortes-Surdilla, “and I always knew that I wanted to come back home to serve my community.” As a family medicine practitioner, she believes that each person has an important role to play in their family’s health. “I love watching families grow and change,” she says, “and I’m honored that I get to assist them along the way.” Dr. Cortes-Surdilla has special experience helping young adults transition from pediatrics to adult medicine.

Dr. Reshma Gandhi, MD

“I love family medicine,” says Dr. Gandhi, Axis’s Associate Medical Director, “it gives you the chance to really get to know people.” Coming from a family that has faced its own medical struggles, she aims to make patients feel supported and informed. She believes that “preventative medicine is the best medicine,” and works with patients to stop disease and symptoms before they start. She is very experienced in treating chronic diseases, especially diabetes.

Dr. Gia Gray, MD

Dr. Gray’s commitment to patient care stems from her upbringing in Chicago, where she developed a passion for caring for those in her community. “I grew up in an underserved community, so I understand the challenges in access to quality of care that many people face.” Dr. Gray focuses on preventive care with her patients, and enjoys working together to find solutions that are effective in reducing conditions such as hypertension.

Dr. Priyanka Kallem, MD

Dr. Kallem is devoted to community clinics, and says that she is “passionate” about Axis’ mission. She usually sees adults and also provides urgent and acute care for children. She particularly focuses on chronic disease management, in order to help her patients’ health improve.

Dr. Cho Cho Lay, MD

Dr. Lay has four sisters – three of whom are doctors! From an early age, their dedication to medicine inspired her to follow in their footsteps. She believes that patients and providers should work together as a team. “We’re on a journey to good health,” she explains, “if the patient is the driver, then I’m the GPS!” Dr. Lay is passionate about community health, and is excited to be a part of the Axis team.

Swechha Shrestha, FNP

Swechha got her start in medicine while working in assisted living. “I got lucky,” she says, “it helped me discover my passion for providing care for others.” With her warm, encouraging style, Swechha aims to make patients feel comfortable sharing their health concerns. “When a patient feels empowered to speak – then we can really work together as a team!” Swechha has a special interest in chronic disease management and looks forward to helping patients achieve their health goals.

Suzanne Silva, MSN, FNP

Suzanne seeks to build trusting partnerships with her patients through conversation – both in English or Spanish. This helps her tailor treatment plans and connect patients with other Axis program. Before joining Axis, Suzanne volunteered with medical programs in many different countries, where she gained special experience working with people with diabetes, dietary problems, and cardiac issues.

Andrew Tang, MSN, NP

“I became a nurse practitioner because I want to help people achieve whole-person health,” says Andrew Tang. Specializing in Family Medicine, Andrew has a passion for partnering with his patients to prevent diseases or help manage them. He encourages patients to ask questions and to be active participants in their health. “Many of my patients don’t speak much English. I hope to encourage them to come to the clinic for treatment and not to be afraid of the language barrier.”

Priyanka Thapa, FNP

Priyanka grew up in Nepal before moving to the Bay Area for high school. That experience shaped her desire to help people, especially those who need access to health care. “It is with pride and pleasure to be doing something good for the community I live in. It brings me lots of happiness.” Priyanka has experience working with transgendered and immigrant populations, as well as urgent and primary care. When it comes to providing care, she believes the best course of treatment comes from listening closely to what patients have to say.

Dr. Sadaf Zafer, MD

Dr. Zafer’s father, who was also a doctor, inspired her to work in healthcare. She chose family medicine because she values its focus on treating each patient as a whole person, and its emphasis on long-term doctor-patient relationships. In the exam room, she addresses a wide variety of health issues, and specializes in managing diabetes, asthma, high cholesterol, and high blood pressure.

Tara has a passion for helping her patients overcome challenges like pelvic pain and infertility. She chooses to ease her patients’ health concerns by empowering and educating them. Tara takes pride in working at Axis, where she feels like she can make a big impact on her patients’ lives and health outcomes.

Elizabeth Valjalo, MSN, NP

Taking care of others is second nature to Elizabeth, who has helped with her mother’s care since she was young. Elizabeth has years of community health center experience, and is excited to be working with patients in her own community.

She particularly enjoys working with newborns and first time parents, and makes time in every appointment to address parents’ concerns during this exciting and challenging time.

Dr. Armani is a great resource for Axis patients who want to learn about how food impacts their health. She usually sees patients who are diabetic, overweight or underweight, have high cholesterol, or have digestive issues. She works with patients to find ways to fit healthy food into their budgets and lifestyles, and supports them as they change their eating habits. She’s thrilled when her patients succeed at their goals, whether it’s losing weight, increasing energy, controlling blood sugar, or using less medication.

Dr. Dhiren Nanavati, MD

FEES

We accept Medicare, Medi-Cal and many public insurance plans. Some services are available on a sliding scale based on income and family size.

ENROLLMENT

If you do not have insurance or the resources to pay for your services, we will work with you to make sure you get the care you need. No one will be denied access to services due to inability to pay. Call 925-462-1755 to schedule an enrollment appointment.
