What can Axis’ Enrollment Specialists do for me?
Axis Enrollment Specialists assist uninsured and/or low-income individuals in determining their eligibility for many State or Alameda County programs for free or low-cost health services, such as Medi-Cal and Covered California. Enrollment Specialists also assist with applications for Cal-Fresh, a supplemental nutrition assistance program (SNAP), Cancer Detection Program (CDP), Family Pact, and others. Enrollment Specialists assist with yearly renewal applications as well.
What do I need to bring for my Enrollment appointment?

How can I get my medical records?
Patients can request their records a few different ways:

- The easiest way to access your Medical Records is through MyChart
- To get a Medical Record release form, call (925)462-1755 and press 6 to leave a message

- Secure Email
- Pick-Up
- Fax
What type of insurance do you accept?
We accept Medicare, Medi-Cal, Health Program of Alameda County (HealthPac), Contra Costa Health Plan, Covered California Blue Shield, and other public insurance plans. Services are also available on a sliding scale based on income and family size. No one will be denied access to services due to their inability to pay.

What if I don’t have health insurance?
If you do not have insurance or the resources to pay for your services, we will work with you to make sure you get the care you need.
Does Axis offer walk-in services?
Most of our services are by appointment only. We offer walk-in service for TB tests and pregnancy tests at our medical sites.
Do I need to bring anything for my medical appointment?
Please bring your state or federally issued photo identification card and any medications you are currently taking to your next medical appointment.
What if I need care after hours or on the weekends?
If you need after-hour care or advice, you can call our main number at 925-462-1755.
You will be able to choose from the following options:
Option 1- Connects you to an operator. The operator will contact an on-call provider who will then call you back.
Option 2- For prenatal patients with medical concerns. You will be connected to Stanford Health Care – ValleyCare labor and delivery services.
What if I need a prescription refill?
Please call your pharmacist to request a refill. You can assist your pharmacist and avoid any delays by calling your pharmacy a few days before you run out of your medication. This will give your pharmacist time to contact your doctor for authorization to refill your prescription if all refills have been used.
