Axis is excited to open our newest clinic and medical hub! It features medical, dental, mental health, and enrollment services, designed to be under one roof to better serve our patients.  The Axis Care Team is committed to ensuring a smooth transition and will continue to provide the same high-quality medical care to all our patients.

If you are a current Axis patient, you will receive communication via a letter or text message that your medical or dental provider has moved to this location. Starting January 6, 2025, any upcoming in-person appointments you schedule will be at this location and there will be no interruption in your care.
