Who We Are
Axis Community Health is a nonprofit community health center that provides affordable, and compassionate health care services that promote the well-being of all members of our community. We have served eastern Alameda County, known as the Tri-Valley, since 1972, and have grown over the years to become a vibrant organization with 200 employees and five sites. We have been designated by the US Department of Health and Human Services as a Federally-Qualified Health Center, and we are also a nationally-recognized leader in clinical excellence.
Our Locations
Axis has five locations throughout the Tri-Valley in the East Bay portion of the San Francisco Bay Area. We are centrally located 25 miles from San Francisco, 15 miles from Oakland and 15 miles from San Jose. The Bay Area Rapid Transit system (BART) is in our community, and offers easy access throughout the Bay Area. The Tri-Valley is notable for its high quality schools, a multitude of parks, hiking trails and soccer fields, and for being in the heart of wine country. We enjoy four beautiful seasons and get to enjoy our beautiful outdoors year round.
