Enrollment Services
Our enrollment team is available via telephone, and in-person Monday – Friday to help patients enroll in Covered CA, Medi-Cal, CalFresh, Medicare, and HealthPAC. Axis also welcomes patients who are looking to pay for all their services. Please call 925-462-1755 to make an appointment.
In order to complete the enrollment process without contact, patients can transmit documents to Axis via a secure, encrypted link that Axis will provide you with. You will need to provide your last pay stub to verify year to date salary amount and if receiving unemployment, an award letter to enter the current income. You may also drop off any necessary documents in person as directed by our Enrollment Staff. After we receive your documents, a Certified Enrollment counselor will follow up with a phone call to verify your information.

Sliding Scale Fee
Axis offers sliding scale discounts for eligible low income patients. To learn more, please make an appointment with our Patient Services Department. No one will be denied health care based solely on their inability to pay.
No Surprises Act
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the “No Surprises Act” law, all health care providers are required to give an estimate of any medical bills to self-paying patients (uninsured patients). Axis will send you an estimate in writing or via MyChart at least 1 business day before your appointment. You can also ask Axis for an estimate ahead of time or prior to any appointments you make. If you receive a bill that is at least $400 more than your estimate, you can call Axis to discuss the bill. Covered California
Health Insurance options are changing fast. New health insurance laws will affect different people in different ways. Axis Community Health’s Certified Enrollment Counselors can answer your questions about health insurance and help get you covered.
Medi-Cal enrollment is open all year. Medi-Cal is California’s Medicaid program. Our Enrollment Counselors can help new patients apply for Medi-Cal, and help current Medi-Cal patients renew their coverage.
Covered California may offer low-cost options. If you are not eligible for Medi-Cal, you may be able to get coverage through Covered California. We can help you fill out your online application and help determine if you are eligible for financial assistance to cover the cost of a health plan.
Individuals who experience a major life event may qualify for “Special Enrollment,” and enroll within 60 days of this life event. Common examples of qualifying events include marriage or entry into a domestic partnership, the birth of a child or adoption, job loss, change in immigration status, relocation to or within California, and many other situations. CalFresh
CalFresh is a nutrition program that helps families get healthy food. When a family or individual signs up, they receive a standard Electronic Benefits Transfer (EBT) card that they can use to buy food at grocery stores, markets, and some online food delivery platforms. Every month, the card is reloaded with a specific amount based on the family’s size and income.
