Health Connect by Axis, is remote patient monitoring program offered at Axis Community Health. This program is designed to monitor the blood pressure of patients with hypertension or the blood sugar or patients with diabetes. This program requires participant’s consent to enroll in the service. Ask your provider about Health Connect at your next appointment.
How will this service help you manage your chronic health conditions?

- When you are enrolled into Health Connect, you will be able to share your blood pressure or sugar readings from home through text messages.
- This data will be shared with a Health Connect nurse who monitors your readings on a regular basis.
- If your readings are concerning, the nurse may call to check on you and will notify your primary care provider to work on the next steps of care.
What should you know about this service?

- Once your provider refers you to Health Connect, you will get a phone call from Axis in the next 7-10 days and asked to be enrolled in this program.
- If you need a blood pressure cuff, Axis will mail you a BP monitoring device, which takes 2-3 weeks to arrive.
- Once enrolled, you will get a text message 3 days a week – Monday, Wednesday, and Friday – at 8 AM. Please reply as soon as you can. If you miss a day, that’s ok!
- After two weeks of sending your readings, your provider will receive a report of your progress!
