IBH CARE COORDINATORS
Cindy Nava
Cindy is Axis’s Lead IBH Care Coordinator and has been with Axis since 2009. She feels that the most rewarding part of her job as a Care Coordinator is the connections she builds with her patients and community. When asked what she loves most about working at Axis, she replies, “I love the growth opportunities Axis has provided me and being part of a great team that supports each other and the community”.
Ashley Bhatia
Ashley is the Behavioral Health Liaison and MAT (Medication-Assisted Treatment) coordinator. She loves supporting patients in their personal growth and progress toward their goals. She also enjoys learning new resources and ways to better assist our patients. What I love about Axis is the connectedness and the effort the agency does to appreciate its staff.
Furoghany Mosley
Furoghany is the IBH Care Coordinator for Pediatrics at Axis. Her hopes for her patients are to meet their needs and to help them get high-quality care. She says, “In the short time I have been with Axis, I have felt very welcomed and supported by the IBH team”. She hopes to help patients understand the importance of mental health.

“I have been seeing counselors at Axis Community Health for 8 months now on a weekly basis. The concern and kindness they show, even just asking, “how was your day?” really means a lot when you don’t hear those words often. The best thing is that they want to see you succeed and they are willing to do everything they can to help you through even your worst ordeals.”
